胃肠道间质瘤的CT表现

CT findings of gastrointestinal stromal tumors

目的探讨胃肠道间质瘤(GIST)的CT表现。方法回顾性分析经手术病理证实的GIST23例,术前均经CT平扫和增强扫描,复习CT表现并与手术病理对照。结果23例GIST中原发于胃17例,十二指肠1例,小肠3例,肠系膜1例,直肠1例。CT表现为内生性、外生性或混合性生长的肿块;体积较小的GIST,密度均匀,边界清楚,增强扫描均匀强化;体积较大的GIST,密度不均匀,肿瘤内常见坏死及囊变,增强扫描不均匀强化。CT术前诊断GIST良恶性的准确率为82.6%(19/23)。结论GIST的CT表现有一定特点。CT可作为术前诊断GIST的主要检查方法。

Objective To explore the CT findings of gastrointestinal stromal tumors（GIST）. Methods All 23 cases patients with GIST conformed by surgical pathology underwent spiral CT plain scanning and two phases dynamic enhancement scanning before operation.The CT findings were reviewed and analysed restrospectively in comparison with surgical and pathological results. Results Among total 23 cases of GIST, there were 17 originated from stomach and 1,3,1,1 from duodenum, small bowel, mensentery, and rectum,respectively. CT findings were endophytic or exophytic or endo-exophytic tumors. Small size tumors were appeared with equable density, smoothing border lines and equable reinforcement after enhanced CT scanning; while large size tumors were appeared with inequable density, necrosis and cystoid changes in the center, unsmoothing border lines and inequable reinforcement after enhanced CT scanning. The concordance rate for CT with pathological results was 82.6%. Conclusion GIST has some features of CT findings. CT is the main tool in the preoperational diagnosis of GIST.
