宾赛克嗪对胆碱酯酶抑制剂诱发犬循环衰竭和普鲁卡因胺诱发犬心源性休克的治疗作用 被引量:4

摘要 目的在胆碱酯酶抑制剂塔崩诱发犬循环衰竭模型和普鲁卡因胺诱发犬心源性休克模型上,评价新药宾赛克嗪的治疗学作用。方法塔崩中毒循环衰竭模型:3只健康成年♂杂种犬,体重12~18kg。在麻醉状态下,肌肉注射塔崩,直至循环衰竭,观察循环衰竭前后血流动力学参数、心电图的变化以及宾赛克嗪0.1mg·kg-1的治疗作用。普鲁卡因胺诱发犬心源性休克模型:4只健康成年♂杂种犬,体重12~15kg。在麻醉状态下,静脉快速推注普鲁卡因胺100mg·kg-1,直至循环衰竭,观察循环衰竭前后血流动力学参数、心电图的变化以及宾赛克嗪0.1mg·kg-1的治疗作用。结果2种休克模型,在循环衰竭时,收缩压(SBP)、舒张压(DBP)、平均动脉压(MBP)、心率(HR)、反映心脏收缩功能的心室内压最大上升速率(+dp/dtmax),反映心脏舒张功能的心室内压最大下降速率(-dp/dtmax)和心肌纤维缩短速度(Vpm),均明显下降;左室舒张压(LVDP)和等容收缩期压力(IP)明显增高。塔崩诱发犬循环衰竭模型,宾赛克嗪0.1mg·kg-1救治后1min,与循环衰竭时相比,血流动力学指标及心电图开始明显改善。普鲁卡因胺诱发犬心源性休克模型,宾赛克嗪0.1mg·kg-1救治后10min,血流动力学指标开始明显改善,60min后心电图才明显改善。结论宾赛克嗪对胆碱酯酶抑制剂诱发的循环衰竭具有强效和速效治疗作用,优于对普鲁卡因胺诱发的心源性休克的治疗作用,表明其抗循环衰竭作用具有特异性。 Aim To investigate the therapeutic effects of benthiactzine against circulatory failure induced by tabun and cardiogenic shock by procainamide in dogs. Methods Tabun was used to induce circulatory failure shock model. 3 healthy male dogs, weighing 12 - 18 g,were treated with tabun (im.) to induce circulatory failure shock in which the mean blood pressure (MBP) decreased to 5.98 kPa. Procainamide was used to induce shock model. 4 healthy male dogs, weighing 12 -15 g,were treated with procainamide (iv) 100 mg·kg^-1 to induce circulatory failure shock, then these dogs were treated with benthiactzine 0. 1 mg·kg^-1(im). The changes of hemodynamic parameters and cardiograms were observed. Result In circulatory failure dogs, SBP, DBP, MBP, HR, LVDP, + dp/dt - dp/dtmax and Vpm changed dramatically ( P 〈 0. 01 ). The hemodynamic parameters and results of cardiograms were improved within 1 min treatment of benthiactzine in tabun-induced circulatory failure shock model. In procainamide-induced circulatory failure shock model, the hemodynamic parameters were markedly improved,while the effect on cardiograms showed 60 min later. Conclusions Benthiactzine could be a more favourable medicine against circulatory failure induced by tabun instead of procain amide to save time for other clinic treatments.
