低剂量CT扫描在学龄前儿童副鼻窦检查中的应用研究 被引量:6

摘要 目的探讨学龄前儿童鼻窦低剂量CT扫描技术及其临床的应用价值。方法将60例正常或轻度副鼻窦炎患者按扫描参数随机分为6组,每组各10例。第1~3组电压120kV,电流分别为100mAs、50mAs、30mAs;第4、5组电压100kV,电流分别为50mAs、30mAs;第6组电压80kV,电流30mAs。用Kruska-WallisH检验和Nemenyi法检验比较各组图像质量有无差异。结果第2~6组的权重CT剂量指数(CTDIw)与第1组的CTDIw比较分别减少50%、70%、78%、87%、92%;第2~4组与第1组比较所得图像的质量评分之间无显著性差异(χ2=0.0905、0.5694、6.2862,P>0.25);第5、6组与第1组比较所得图像的质量评分之间有显著性差异(χ2=19.0531、53.5293,P<0.01)。结论学龄前儿童鼻窦低剂量CT扫描参数低至50mAs,100kV,CTDIw2.53mGy时可满足临床诊断要求。 Objective To study the usefulness of low-dose CT scan technique in examining paranasal sinus of preschool children. Methods Sixty subjects with normal paranasal sinus and mild paranasal sinusitis were randomly divided into 6 groups (each group 10 cases) according to different scan parameters. Group 1,2,3 were scanned by 120 kV,100 mAs,50 mAs,30 mAs; group 4,5 by 100 kV,50 mAs, 30 mAs,and group 6 by 80 kV, 30 mAs under other scanning parameters to be unchanged.lmage quality of every group was compared by using Kruska-Wallis and Nemenyi test to see whether there was difference between varied groups.Results compared with groupl,CTDIw of group 2 to 6 were reduced by 50%,70%,78%,87% and 92%,respectively. No significant difference was found between group 1 and group 2,3,4(X2 =0.0905,0.5694,6.2862,P〉0.25) but there was significant difference between group 1 and group 5,6(x2 =19.0531,53.5923, P〈0.01).Condusion In the paranasal sinus examinations of preschool children, low-dose CT scan with 50 mAs and 100 kV (2.53 mGy) can satisfy the diagnostic requirements.
