肠系膜损伤的CT、B超回顾分析(附8例报告)

Evaluation of mesenteric injuries with CT and US:a retrospective study of 8 cases

目的探讨肠系膜损伤的CT、B超诊断价值。方法回顾分析8例经过手术证实肠系膜损伤的CT平扫资料,其中,4例有急诊B超资料。所有病例均因车祸入院。结果CT表现包括肠系膜密度增高8例,肠间积液6例,腹膜后积液3例,肠系膜血肿2例,肠壁增厚2例,3例表现为"夹心样"肝脏周围、脾脏周围积液。B超检查2例显示大量腹腔积液,2例少量腹腔积液。结论CT对于肠系膜损伤的诊断具有很好的敏感性,B超显示腹腔积液而无实质性脏器损伤时,高度提示肠系膜损伤可能。

Objective To determine the value of computed tornography（CT） and ultrasound （US） for detecting mesenteric injuries secondary to abdominal blunt trauma.Methods Eight patients with surgically proved mesenteric injuries underwent CT plain seans. Of whom, 4 patients underwent additional ultrasound examinations. The imaging data in all patients were analyzed retrospectively.Results CT showed mesenteric hyperdense in all patients,of whom,six cases had inter-bowel fluid, three cases had retroperitoneal fluid, two cases had mesenteric hematoma ,two cases had bowel wall thickening, and three cases had perihepatic and perisplenic sandwich-like fluid.In 4 pa- tients who underwent US,a large amount of pyoperitoneum and small amount of pyoperitoneum were found in 2 cases, respectively.Con- clusion CT is highly sensitive in the diagnosis of mesenteric injuries.It suggests highly mesenteric injuries that ultrasound showed a large amount of pyoperitoneum without parenchymal organ injuries.
