利用RIA法测定了经皮层EEG、PET、SPECT、MRI等检查定位明确的68例癫痫患者的基础GH水平和癫痫发作后血清GH水平。结果发现,癫痫患者基础GH水平(1.99±1.64μg/L)明显低于正常对照(p<0.01)。发作后GH水平明显升高,发作后30min,64.7%的病例GH分泌达到峰值(8.15±4.93μg/L),约为发作前基础水平的4倍。发作后90min,89.7%的病例GH水平下降,其中60.3%的病例GH水平下阵至正常或发作前基础水平。发作后GH水平的变化与癫痫发作类型、病灶位置无关,均可引起发作后GH水平的升高。但发作前后GH水平的变化与SPI的长短有显著的关系,SFI越短,基础GH水平越低,发作后GH水平升高的幅度越小。结果提示,癫痫过程明显影响垂体GH的分泌,癫痫反复发作可导致垂体的GH储备下降,使基础GH水平低于正常。
We measured serum basal and post-seizure attack levels of growth hormone (GH) in 68 epileptic patients whose epileptic foci were defined by cortical EEG,PET,SPECT and MRI examinations. The results showed that the basal levels of GH were significantly lower in epileptic, patients than those in controls (mean basal value 1.9.9±1.64μg/L, p<0.01). The GH levels increased significantly after seizure attacks, reaching peak at 30 minutes in 64.7% of the patients (8.15±4.93μg/L, about 4x basal level). At 90 minutes, the post attack levels were decreased in 89.7% of the patients, in which 60.3% approaching control or basal levels Changes of post-attack GH levels bore no specific relation to the types and foci of the disease but varied significantly with different length of seizure free intervals (SFI). The sborter the SPI was, the lower the basal GH level and the less post-seizure increase. The results sugge-sted that epilepsy had significant influence upon pituitary GH secretion. Re-peated seizure affacks led to decrease of the pituitary GH reserve and lower than normal basal GH values.
