小剂量泼尼松联合异维A胶丸治疗聚合型痤疮临床观察

Clinic Effect Analysis for Conglobata Acne Treated by Isotretinion Combined with Low Dose Predisone

目的探讨小剂量泼尼松联合异维A胶丸治疗中重度囊肿性痤疮的疗效。方法采用随机开放对照研究,入选患者78例,随机分为3组,C组予异维A胶丸0.5～1.0mg/(kg.d)和罗红霉素分散片300mg/d口服,A组在C组基础上同时予泼尼松20mg晨服,3周后泼尼松减为隔日晨服;B组在C组基础上口服吲哚美辛25mg3次/d,共5天,间隔5天重复使用,疗程均为6周。结果A,B,C三组痊愈率分别为74.07%,38.46%,32.00%,差异有显著性意义(P<0.05);有效率分别为92.5%,92.31%,88.00%,差异无显著性意义(P>0.05)。三组均未发现不可耐受副作用。结论小剂量泼尼松联合异维A胶丸治疗中重度囊肿性痤疮可以缩短疗程,提高痊愈率,是一种安全有效的方法。

Objective To probe the clinic effect of low dose predisone combined with isotretinion in treating mild to severe conglobata acne. Methods Randomly opening - control placebo trial was enrolled to 78 patients with mild to serve conglobata acne who were divided into three groups randomly（ A,B,C group）. All patients were administrated isotretinion at dose of 0.5mg - 1mg/kg· d^-1 （40mg daily maximum） orally combined with roxithromycin 300mg daily for six weeks, with addition of predisone （20mg daily oral in the morning in the first three weeks and then decrease to 20mg every two days） patients in group A and indomethacin （25 mg three times daily for five days weekly for six weeks） in group B. Group C was taken as control. Results After six weeks therapy course, cure rate for group A to C was 74.07%, 38.46% 32.00% respectively, and had significantly statistics difference （P 〈0.05）, but effect rate of them was 92.5% ,92.31% and 88.00% respectively and had no significantly statistics difference. Conclusion Isotretinion combined with low dose predisone can shorten therapy course and improve cure rate in treating mild to severe conglobata ache. It was safety and a alternative therapy in severe acne treatment.
