舌侧矫治技术临床应用初探

目的:研究临床应用舌侧矫治技术这一美观的牙齿矫正方法的技工室准备、临床操作、患者的适应性、矫治效果等。方法:两例安氏Ⅱ类Ⅰ分类成年女性患者,不拔牙采用舌侧矫治技术。参照C.L.A.S.S.(Custom Lingual Application Setup System)间接粘结方法进行托槽和磨牙舌侧管的粘结,观察记录患者对矫治器的适应性及矫治效果。结果:间接粘结的技工室操作部分比较复杂、费时,但临床口内粘结过程作者感觉并不比唇侧托槽的粘结困难。矫治初期患者感觉说话、咀嚼困难,约两周之后慢慢适应。2个月后前牙深覆得到矫正,11个月后主动矫治结束。结论:在医师的指导下,患者可以在较短的时间内适应舌侧矫治器。间接粘结法虽然比较复杂,但其具有粘结位置准确、临床粘结耗费时间短等优点。由于舌侧矫治技术具有不可替代的美观性优点,将会有越来越多的患者和正畸医师青睐和应用这一矫治技术。

Objective To study the method of indirect bonding that should be used in lingual orthodontics, the clinical operations and the cautions. Methods Two Class Ⅱ division Ⅰ female adult patients were treated in lingual orthodontics. Lingual brackets and tubes were bonded according to the C.L.A.S.S. system. The discomforts of the patients and the effects were observed. Results The laboratory techniques of indirect bonding were relatively complex, but the clinical part of bonding was not more complex than the labial direct bonding. The patients adapted the braces in about two weeks. The overbite was corrected in two months and the active treatment was finished after 11 months. Conclusions Patients could adapt the braces in a short time under the guidance of the doctor. The locations of brackets bonded by indirect method were more accurate and the brackets could be bonded easily and rapidly in clinic. More and more orthodontists and patients will use the technique of lingual orthodontics in the future because of its invisible advantage.
