骨皮质剥脱术治疗四肢长骨干骨不连的疗效观察

Outcome of decortication in treatment of bone shaft nonunion in extremities

目的探讨骨皮质剥脱术对四肢长骨干骨不连的治疗作用。方法采用骨皮质剥脱术治疗53例（58根长骨）骨不连，其中7例患者伴有肢体畸形，38例患者同时接受骨移植手术；观察骨愈合时间、畸形矫正及功能恢复情况。结果随访3～24个月，平均13个月。患者均1次手术成功，骨愈合时间3～12个月，平均6个月，均经X线或CT证实。7例患者肢体畸形得以矫正，骨不连部位的皮肤无坏死。患肢功能均恢复良好。结论骨皮质剥脱术是治疗骨不连安全、有效的方法。

Objective To explore the outcome of decortication in the treatment of bone shaft nonunion in extremities. Methods Fifty-three patients with 58 bones shaft nonunion were treated with decortication, among whom 7 were complicated with limb deformity and 38 received bone grafting. The healing time, malformation correction and functional recovery were observed. Results The time of follow-up was 3 to 24 months, with an average of 13 months. Bone union was approved by X ray or CT. The healing time was 3 to 12 months, with an average of 6 months. Malformation correction was achieved in 7 patients, with no skin necrosis at the site of bone nonunion. The functions of affected extremities recovered well. Conclusion Decortication is a safe and effective method in the treatment of bone shaft nonunion.
