38例下肢深静脉血栓彩色多普勒超声诊断的体会

Experiences on 38 cases of lower-extremity deep venous thrombosis diagnosed by color Doppler ultrasound

目的提高彩色多普勒超声检查诊断下肢深静脉血栓形成的水平。方法结合临床和相关文献对彩色多普勒超声检出的38例下肢深静脉血栓的图像特征进行回顾性分析。结果按起病方式有急性血栓12例,亚急性血栓21例,慢性血栓5例。按发病部位有左下肢23例,右下肢11例,双侧4例。按血栓分布有中央型24例,周围型8例,混合型6例。按栓塞程度有完全性栓塞13例,不完全栓塞22例,再通性栓塞3例。所有病例均经临床治疗证实,其中29例经X线静脉造影证实。有2例超声检查与X线造影不符合。结论彩色多普勒超声检查对诊断下肢静脉血栓形成的部位及栓塞程度的判断有独特的优越性,具有无创、实时、准确及可重复性强等优点,是下肢深静脉血栓的首选检查方法。

Objective To raise the level of diagnosis of lower-extremity deep venous thrombosis by color Doppler ultrasound. Method Retrospective analysis about image feature of 38 cases of lower-extremity deep venous thrombosis diagnosed by color Doppler ultrasound was made combined with clinic and pertinent literature, Results There were 12 cases of acute thrombosis, 21 cases of subaeute thrombosis and 5 cases of chronic thrombosis according to their style of onset. There were 23 cases in left lower extremities, 11 cases in right lower extremities and 4 cases in extremities of both sides according to the site of thrombosis. There were 24 cases of central type, 8 cases of peripheral type and 6 cases of mixed type according to the distribution of thrombosis. There were 13 cases of completer embolism, 22 cases of incomplete embolism and 3 cases of embolism after recanalization. All the cases were confirmed by clinical treatmeent, among which 29 cases were confirmed by x-ray phlebography and 2 cases had disprepaney between ultrasonic inspection and contrast x-ray. Conclusion Color Doppler ultrasound has distinct superiority as to diagnosing lower extremity thrombogenie site and judging the extent of embolism. It＇s the preferred method to inspect lower extremity deep venous thrombosis because it is noninvasive, real-time, accurate and of good repeatability.
