老年胃癌术后早期肠内与肠外营养的应用比较

Comparison of Postoperative Enteral Nutrition and Parenteral Alimentation for Gastric Carcinoma in the Elderly

目的探讨老年胃癌患者术后早期营养支持方式及疗效。方法78例老年胃癌患者随机分为两组,肠内营养(enteral nutrition,EN)组40例与肠外营养(parenteral nutrition,PN)组38例。EN组均采取术中放置十二指肠营养管,术后第2 d开始行肠内营养。PN组术后第1 d开始行中心静脉营养支持。结果两组患者在术后并发症方面无明显差异,但EN组术后住院日、住院总费用、营养相关费用等卫生经济学指标明显优于PN组(P<0.05)。结论老年胃癌患者术后早期营养支持有利于机体恢复,与PN组相比,EN具有方便、安全等优点,且性价比高,值得推广应用。

Objective To discuss the styles and efficacies of postoperative early nutrition for gastric carcinoma in elderly. Methods 78 geratic cases of gastric carcinoma were randomized into 2 groups, including 40 of enteral nutrition（EN） group, and 38 of parenteral nutrition（PN） group. In the EN group, the nutrient canal was put into the duodenum intraoperatively and the EN started from the second day after operation. In the PN group, the central vein nutrition was used from the first day after operation. Results There was no difference of the postoperative complication in the two groups. But the hospital stay, total cost and the nutriton correlated cost of EN gained advantages to the EN group. Conclusions The early nutritional support is helpful for the geratic gastric carcinoma in the aspect of postoperative recovery. EN has the advantages of convenience, safe and high effect/cost ratio, and is worthy to be popularized.
