腹腔镜与剖腹手术治疗卵巢良性囊肿疗效比较

Effect Comparison of Laparoscopy with Laparotomy for Benign Ovarian Cyst

目的探讨腹腔镜手术治疗卵巢良性囊肿的价值。方法卵巢良性囊肿行腹腔镜手术(腹腔镜组)84例,同期行传统开腹手术96例(开腹组),比较两组的临床效果。结果腹腔镜组除l例中转开腹外,余均顺利完成手术,无术中术后并发症。腹腔镜组和开腹组术中出血量分别为(20.0±1.3)ml,(45.0±2.7)ml;手术时间分别为(60.0±3.5)min,(75.0±4.7)min;下床活动时间分别为(5.7±2.2)h,(24.2±3.4)h;排气时间分别为(8.2±2.5)h,(26.3±10.3)h,拔除尿管时间分别为(10.3±3.8)h,(28.6±8.9)h;住院时间分别为(6.0±1.8)d,(8.0±3.2)d。两组比较差异有显著性(P<0.05)。腹腔镜组术后体温升高9例,开腹组均升高,组间比较差异显著(P<0.05)。结论腹腔镜手术具有损伤小、疼痛轻、恢复快等优点,是目前治疗妇科卵巢良性肿瘤的最佳手术方式。

Objective To explore the clinical vale of laparoscopy in treatment of benign ovarian cyst. Methods 84 cases of benign ovarian cyst were performed laparoscopy, and 96 homochronous cases were carried out open surgery. The clinical effects were compared between the two groups. Results Except one case conversed to open surgery, 83 cases were completed the laparoseopy successfully without intra- and post-operative complications. The mean operation duration, intra-operative blood loss, off-bed activity time, exhaust time, removal time of urinary catheter, and hospital stay was （60. 0 ±3.5） min vs. （75.0 ±4. 7） min, （20.0 ±1.3 ） ml vs. （45.0±2.7）ml, （5.7±2.2）hvs.（24.2±3.4）h, （8.2±2.5）hrs. （26.3 ±10. 3）h, （10.3±3.8）h vs. （28.6 ± 8.9） h, and （6.0 ± 1.8 ） d vs. （ 8.0 ± 3.2 ） d in laparoseopic group and open surgery group, respectively. There was significantly difference between two groups （ P 〈 0.05 ）. Post-operative fervescenee occurred in 9 eases of laparoseopie group and all 96 eases of open surgery group, the results showed remarkable differenee between two groups （ P 〈 0. 05 ）. Conclusion Laparoseopy could be the best procedure for benign ovarian cyst, with advantage as minimal invasion, mild pain, and rapid reeovery.
