肠引流器在结肠癌梗阻中的应用探讨

目的探讨结直肠癌急诊手术中肠引流器的应用,为一期结肠切除吻合创造条件。方法回顾分析我院自2000年4月-2007年4月之间,结直肠癌并发急性肠梗阻36例病例资料。结果行一期肠切除吻合27例;姑息性结肠造瘘4例。另5例经肛门插入型肠梗阻导管减压去污后行一期手术。术后感染1例,无吻合口瘘,无手术死亡病例。结论自制肠引流器与灌肠管、结肠形成封闭引流及冲洗环境,避免了手术污染。引流冲洗方便,为一期手术创造条件,具有临床应用前景。

Objective To evaluate the use of a kind of drainage pipes in emergent operations in colorectal carcinoma patients and to improve the healing conditions for one - stage colorectostomy. Methods Total 36 cases of colorectal carcinoma with emergent bowel obstruction from 2000 to 2007 in our hospital were analyzed retrospectively. Results Total 27 cases were Underwent one - stage resection and anastomasis, 4 underwent palliative colostomy and 5 underwent one - stage anastomasis after alleviating the bowel pressure by drainage pipes. After these operations, one patient was infected and there was no anastomotic leakage and death case. Conclusion Bowel drainage pipe, enema pipe and colon can make up a closed circumstance and avoid the operational contamination. This kind of bowel drainage pipe was convenient and improving the healing conditions for one - stage colorectostomy, thus may have a good prospect in clinic.
