经纤维支气管镜不同取材三步联检法对周围型肺癌的诊断影响

Value of using fibrobronchoscope of different sample-selected methods combined in three-steps to diagnose peripheral iung cancer

目的探讨经纤维支气管镜不同的取材三步联检法对周围型肺癌诊断的影响。方法通过回顾性分析我院2004-2006年门诊及病房疑诊周围型肺癌应用了盲检、刷检、选择性BALF 3种不同取材三步法取材行纤支镜检查患者，筛选出65例最后确诊为周围型肺癌患者，分别统计盲检、刷检、选择性BALF 3种不同取材三步法对周围型肺癌的检出率。结果盲检、刷检、支气管肺泡灌洗液（BALF）的阳性率分别为41．5％、38．5％、56．9％，联合三种取材方法总阳性率为64．6％。结论经纤维支气管镜检查是周围型肺癌的重要诊断方法，经纤维支气管镜检查对周围型肺癌患者的诊断率与取材方式、技术有密切相关，联合运用多种取材三步联检法可明显提高周围型肺癌的诊断准确率，并且并未增加并发症。

Objective Probe into the diagnosis value to peripheral lung cancer under the branchofiberoscope with different methods combined of drawing materials in three-steps. Methods Adopt retrospective analysis to review the possible peripheral lung cancer patients who is in-hospital or out-hospital from the year 2004 to 2006, bolting 65 patients who had received the three different ways to draw materials including clamp check, brush check and BALF and diagnosed peripheral lung cancer in last. and to statistic the diagnosis rate of peripheral lung cancer by three different ways to draw materials including clamp check, brush check and BALF in three-steps. Results The positive rate produced by clamp check ,brush check and BALF is 41.5% ,38.5% , 56.9% respectively. The total positive rate of the three-methods combined in three-steps is 64. 6%. Conclusion Using fibrobronchoscope of different sample-selected methods combined in three-steps to diagnose peripheral lung canceris one of important diagnosis means of peripheral lung cancer. The diagnosis rate of peripheral lung cancer is close correlate to the method and skill to draw materials. Using different methods combined of drawing materials in three-steps can remarkably elevate the exact diagnosis rate of peripheral lung cancer ; and it does not increase the complications.
