冠状动脉Gensini积分与冠心病患者中医证候相关性研究

Study of Correlation between Coronary Gensini Score and Chinese Medicine Syndromes in Coronary Heart Disease

目的探讨冠心病患者中医证候与冠状动脉Gensini积分的关系。方法对229例冠心病患者依据CRF表内容进行临床调查,同时分析冠状动脉造影Gensini积分与中医辨证分型的相关关系。结果本组229例CHD患者,按虚实辨证分型,其中实证63例,虚证12例,虚实夹杂证140例,证候无法判别14例。将不同证型的冠状动脉病变程度用Gensini积分的形式表达,研究显示,患者Gensini积分呈现虚证>虚实夹杂证>实证的趋势。将215例CHD患者的中医证型分为血瘀证组与非血瘀证组,根据Gensini积分原则分别将两组的冠状动脉病变用积分表达,血瘀证组积分为(44.26±32.668)分,非血瘀证组为(38.20±33.916)分,血瘀证组高于非血瘀证组,但两组比较无显著性差异。结论冠状动脉Gensini积分与中医虚实辨证及血瘀证存在一定相关性,中医八纲辨证、血瘀辨证对冠状动脉病变判断存在一定的指导意义。

Objective To discuss the correlation between Chinese medicine syndrome and coronary Gensini score in coronary heart disease（CHD）. Methods In light of CRF,clinieal investigation was conducted in 229 cases of CHD and the correlation was analyzed between coronary angiography Gensini score and the differentiation of syndromes in Chinese medicine. Results 229 cases of C HD were differentiated into excess syndrome （63 cases） ,deficiency syndrome （12 cases）,mixed syndrome of excess and deficiency（140 cases）and 14 cases hard to be judged in terms of syndromes differentiation of excess and deficiency. CHD severity of different syndromes was expressed by Gensini score. The study showed that the order of Gensini score was as deficiency syndrome〉mixed syndrome〉excess syndrome in tendency. Chinese medicine syndromes of 215 CHD cases were divided into blood stagnation group and non-blood stasis group. In light of Gensini score,the score expression of CHD was （44.26±32.668）in blood stasis group and （38.20±33.916）in non-blood stasis group. The outcome of blood stasis group was higher than that in non-blood stasis group, but without significant difference. Conclusion A certain correlation of coronary Gensini score is existed in the differentiation of excess and deficiency and blood stasis syndrome in Chinese medicine. Differentiation of syndromes according to eight principles and differentiation of blood stasis present certain guidance significance in the judgment of coronary heart disease in Chinese medicine.
