儿童下呼吸道感染超产β-内酰胺酶细菌耐药的临床研究

摘要 目的探讨下儿童呼吸道感染超产β-内酰胺酶(ESBlS)细菌耐药情况以及临床特征。方法对下呼吸道感染患儿痰标本中分离的34株产ESBLs大肠埃希菌和肺炎克雷伯菌进行耐药及临床分析。结果药敏试验显示产ESBLs大肠埃希菌和肺炎克雷伯菌对亚胺培南、美罗培南无耐药性,而对哌拉西林、头孢唑啉、头孢曲松、头孢噻肟耐药率达100%。结论产ESBLs大肠埃希菌和肺炎克雷伯菌存在较严重的多重耐药及交叉耐药,临床应加强监测,依据药敏情况合理用药。 Objective To explore drug resistance and clinical characteristics of extended spectrum β- lactamases (ESBLs)producing bacteria in children with lower respiratory tract infections. Methods 34 strains of ESBLs-producing escherichia coli and klebsiella pneumoniae were isolated from hospitalized patients with lower respiratory tract infections from Dec. 2001 to Jul. 2006. The drug resistance and clinical characteristics were analyzed. Results Antimicrobial sensitivity test showed that ESBLs-producing strained no drug resistance to imipenem and meropenem, and the drug-resistance rates of ESBLs-producing strains against piperacillin, cefamlin,ceftriaxone, cefotaxime were 100%. Conclusion ESBLS producing escherichia coli and klebsiella pneumoniae are seriously multipie drug resistant and cross drug resistant. So more attention shall be paid to monitoring the drug-resistant in ESBLs-producing escherichia coli and klebsiella pneumoniae at our hospital, and the antibiotics shall be rationally administrated according to drug sensitivity.
