169例绝经后阴道出血临床与病理分析

Clinical and Pathological Analysis of 169 Cases with Postmenopausal Vaginal Bleeding

目的探讨绝经后出血的病因、病理类型及诊断方法。方法对169例绝经后阴道出血患者的临床诊断及病理类型进行分析。结果绝经后阴道出血的主要病因分别是炎症(包括阴道炎、宫颈炎、子宫内膜息肉)(24.26%)、功能性子宫内膜(67.46%)、恶性肿瘤(7.1%)。从病理类型分析:功能性子宫内膜引起绝经后阴道出血者最多,绝经10年以上恶性肿瘤的发生率升高(8/30)26.67%。结论绝经后阴道出血临床病因复杂,要重视临床症状及初步检查,明确病因。绝经后阴道出血的年龄越大,绝经年限愈长,恶性肿瘤发生率愈高。

Objective To investigate the pathogenesis,diagnosis and pathologic type in postmenopausal vaginal bleeding. Methods The clinical and pathologic type of 169 cases of postmenopausal vaginal bleeding were analyzed. Results The main causes of postmenopausal vaginal bleeding were as follows：organ inflammation （include vaginitis, chronic cervicitis, endometrial polyp）（24.26% ）,functional endometrial bleeding（67.46% ）, malignant tumor（7.1% ）. Analyzed from pathologic type： The most cause of postmenopausal vaginal bleeding is functional endometrial bleeding,the rate of malignant tumor was elevated in those menopansal duration over 10 years （8/30）26.67%. Conclusion As the causes of postmenopausal vaginal bleeding are complex, we should emphasize clinical symptom and initial check, do these in order to identify diagnosis. The incidence of malignant tumor is higher in senile women with longer duration of menopansal.
