摘要
目的探索Dextroscope在颅底肿瘤个体化手术人路设计及术中对脑血管保护作用。方法64例颅底肿瘤患者随机分成实验组和对照组,每组分别为32例。实验组术前使用Dextroscope系统设计手术,对照组不使用该系统。记录手术切除肿瘤程度,比较两组手术后脑血管损伤并发症,以及术后KPS评分。结果手术全切率:实验组83%,对照组77%;血管损伤并发症:实验组3例,对照组为7例(P〈0.05);KPS评分:实验组91.75±8.1,对照组85.6±9.4(P〈0.05)。结论利用Dextroscope进行颅底肿瘤术前计划,有助于个体化手术入路设计,有利于减少手术中脑血管的损伤,改善手术后KPS。
Objective To study the effectiveness of application of virtual reality technology of Dextroscope in designing individual approaches of patients with skull base tumors and protecting cerebral vessels during operation. Methods 64 patients with skull base tumors in this research were randomized into 2 groups-test group and control group, each with 32 patients. Before operation, images data such as head MRI,MRA or CTA were collected and detailed. Preoperative plan was made by Dextroscope in the test group, but no Dextroscope plan in control group. Resection degree of tumors and cases with complications of cerebrovascular injury in operation and postoperative Karnofsky performance scores ( KPS ) in two groups were recorded and compared. Results The total resection rate of tumors in test group was 83%, while 77% in control group; there were complications related to cerebral vessels injury in three cases of test group, seven cases of control group( P 〈0.05 ) ; KPS on the 6th month follow-up in two groups was: 91.75 ± 8.1 in test group, 85.6 ± 9.4 in control group ( P 〈 0.05 ). Conclusion Application of preoperative plans with virtual reality technology in skull base tumor operation can help design individual approaches, reduce injury of vessels in procedures and improve the postoperative KPS.
