不同级别胶质瘤的癫痫发生率及其临床特征比较

Incidence of Epilepsy in Different Grades of Intracranial Glioma and Its Clinical Characteristics

【目的】了解不同级别胶质瘤的癫痫发生率。【方法】分析本科2004年1年内经手术治疗的胶质瘤癫痫发病情况。【结果】胶质瘤总的癫痫发生率为33.6%(103/307),星形细胞瘤为35.9%(70/195),其中2级以下(含2级)的低级别(高分化)星形细胞瘤发生率为46.5%(53/114),2级以上的高级别为21.0%(17/81),两者比较有显著差异(P<0.005)。低级别的星形细胞瘤其发病的平均年龄比高级别的发病年龄低,病程则低级别的更长。【结论】低级别(≤2级)星形细胞瘤的癫痫发生率比高级别(>2级)的高。胶质瘤的级别越低,其平均年龄就低,级别越高,年龄越大。

[ObjectivelTo study the epileptic incidence of different grades of glioma. [Methods]The epileptic incidence rate of different grades of glioma which were operated in Xiangya hospital in 2004 were analyzed. [ResultslThe total incidence of epilepsy in glioma was 33.6 % （103/307）. The incidence of epilepsy in astrocytoma was 35.9% （70/195）, which the epileptic incidence of grade 2 below （including grade 2） was 46. 5%（53/114） and the high grade（high differentiation） of grade 2 above （ not including grade 2） was 21.0% （17/81）. The grading of astrocytoma and epileptic incidence had not significant correlation, but the low-grade astrocytoma （high differentiation） had a higher epileptic incidence. The epileptic incidence of the high grade and low grade of astrocytoma had significant difference（ P 〈0. 005）. The age of lower grade of astrocytoma was younger than that of the higher grade of astrocytorna. [ConclusionlLow-grade astrocytoma has higher epi- leptic incidence, and the average age turns older with the increasing of grade.
