212例染色体正常的无精子症患者临床研究

Clinical study on 212 azoospermia patients without chromosomal abnormality

目的了解染色体正常的男性无精子症患者的发病情况,探讨其病因和诊断、处理。方法入选染色体正常的无精子症患者212例,从年龄、职业、身高、体重、腮腺炎病史、棉籽油服用史、睾丸大小、激素水平、睾丸活检等15个方面分析总结流行病学和临床资料。结果在男性无精子症病因中,腮腺炎病史、棉籽油服用史、附睾炎史所占比例较高。在睾丸体积<12ml且FSH明显升高患者,睾丸活检未发现有成熟精子。结论预防腮腺炎的发生,避免食用棉籽油和预防附睾炎发生可以减少部分无精子症的发生。睾丸大小、质地与性激素水平对临床判断无精子症的类别有重要的意义。

Objective To summarize clinical characters of 212 azoospermia patients and investigate its etiology diagnosis and treatment. Methods Total of 212 azoospermia patients without chromosomal abnormality were retrospectively analyzed in the study according to age, occupation, height, weight, mumps, epididymitis, cotton seed eating history, testis size etc. Results Mumps, eating cotton seed oil and epididymitis account for high proportion in common cause of azoospermia patients. For patients with testis volume less than 12ml and elevated level of FSH, no mature sperm was found by testis biopsy. Conclusion Prevention of mumps and epididymitis, eating cotton seed oil might effectively reduce azoospermia. The size and texture of testis and level of hormone were very important for diagnosis of azoospermia.
