低机械指数子宫输卵管超声造影与腹腔镜通染液检查评价输卵管通畅性的对照研究 被引量:6

摘要 目的探讨低机械指数子宫输卵管超声造影对输卵管性不孕的诊断价值。方法运用低机械指数子宫输卵管超声造影技术对24例临床疑为输卵管因素引起的不孕症患者(原发不孕7例,继发不孕17例)进行检查。实时观察造影剂在宫腔及输卵管腔内流动过程,判断输卵管的通畅性。其结果与腹腔镜下通染液检查对照。结果低机械指数子宫输卵管超声造影与腹腔镜下通染液检查诊断输卵管通畅性结果比较,差异无统计学意义(χ2=0.425,P=0.514)。低机械指数子宫输卵管超声造影在评价输卵管通畅的敏感性、特异性、阳性预测值、阴性预测值分别为90%、100%、100%、83%,无明显并发症。结论低机械指数子宫输卵管超声造影诊断输卵管通畅性准确、简便、无创、费用较低,具有临床推广应用价值。 Objective To explore the diagnostic value of low mechanical index hysterosalpingogram in tubal infertility. Methods Twenty - four patients(7 cases of primary infertility and 17 cases of secondary infertility) were examined with low mechanical index hysterosalpingogram. Real - time assessment of tubal patency was conducted by monitoring motion of contrast agent in the endometrial cavity and tubal lumen, then the results were compared with that of laparoscopy and dyeing. Results There was no significant differences between diagnosis of tubal patency with low mechanical index hysterosalpingogram and with laparoscopic dyeing (X2 = 0. 425, P = 0. 514). The sensitivity, specificity, positive predictive value and negative predictive value of low mechanical index hysterosalpingogram were 90%, 100%, 100% ,83% respectively. No obvious complications happened. Conclusion Low mechanical index hysterosalpingogram is a reliable, easy, noninvasive and inexpensive method to diagnose tubal patency, it has important clinical application value.
