连续“Z”成形术修复下腹部蹼状瘢痕

The application of continuous flap Z-plasty in repairing webbed scar of abdominal

目的为下腹部蹼状瘢痕的治疗提供一种简单易行的方法。方法对14例下腹部瘢痕患者采用连续"Z"成形术进行手术整复。对于瘢痕严重挛缩的患者,采用仅对挛缩瘢痕切开松解而不切除的手术方法,灵活设计切口,利用挛缩部位已成熟的瘢痕形成较大面积三角形瘢痕瓣来修复创面。结果临床应用14例均取得满意疗效,皮瓣全部成活,蹼状粘连解除。随访6～12个月,皮瓣色泽同周围皮肤,质地柔韧,皮瓣无挛缩,感觉良好,外形满意,活动正常。结论本方法简单可行且效果良好。

Objective For treatment the webbed sear of abdominal provides a sim-ple and easy method. Methods 14 eases suffered webbed sear eontraeture of abdom-inal were repaired by continuous flap Z-plasty. However, for those with severe sear eontraeture, using only incision sear eontraeture release without the removal of surgical methods, within the comprehensive treatment aforementioned, a special incision on the matured sear was designed,which formed sear formation of larger triangular sear flap to repair the wound. Results All eases got good results, all flaps survived satisfactorily, webbed adhesion have been loosened. All eases were followed up for 6 - 12 months, the tinct,feeling and contour of skin flap were fine without eontraeture, function of excretion and activity of lower limbs were normal. Conclusion It is more effective that the combination therapy to webbed sear eontraeture of abdominal.
