摘要
为探讨支气管灌洗液结核PCR检测对下叶肺结核诊断价值。采用支气管灌洗液PCR与结核杆菌培养、涮检、病检以及血中结核抗体、皮肤PPD试验作对比分析。结果显示,结核组支气管灌洗液PCR阳性率80%,涮检涂片36%、活检12%、培养32%。PCR法明显优于其它三种。χ2检验P<0.01,肺结核组涮检涂片阳性、PCR法同样阳性。血清结核抗体阳性率68%,高于皮肤PPD试验阳性率的32%,结核抗体阳性者PCR法均阳性。结论:支气管灌洗液PCR检测是下叶肺结核早期快速诊断的一种有价值检测手段。
Polymerase chain reaction (PCR) was applied to detect DNA of Mycobacterium tuberculosis in 25 TB and 23 NonTB cases,using bronchial lavage fluids from inferior lobes as the sample;the result of PCR measurement was analytically compared with that of conventional procedures.It was found that in the 25 TB cases the positive rates of bronchial acidfast staining,culture,biopsy,and PCR were 36%,32%,12%,and 80% respectively.The sensitivity of PCR was much higher than the other′s(P<001).Acidfast staining was postive just the same as PCR.PCR will prove positive as long as the antibody against TB in blood shows positive;positive rate of the antibody is 68%,being higer than that of skin PPD (32%).Conclusion:PCR measurement of bronchial lavages is a rapid,sensitive,and valuable procedure for the diagnosis of pulmonary inferior lobe tuberculosis.
