机体铁代谢与2型糖尿病关系研究 被引量:7

ASSOCIATION BETWEEN BODY IRON METABOLISM AND TYPE 2 DIABETES

摘要 目的对新诊断2型糖尿病(NEW-T2DM)患者体内铁负荷状态、氧化应激水平及抗氧化状态进行分析研究。方法通过口服糖耐量实验,将所有对象分为正常糖耐量组(NGT)、T2DM前期组(空腹血糖受损/糖耐量异常,IFG/IGT)和新诊断2型糖尿病组(NEW-T2DM)。测定各组血浆中铁蛋白(SF)、转铁蛋白(TF)及血浆铁(Fe2+)含量,以评价机体铁代谢状况;测定血浆丙二醛(MDA)含量和总抗氧化能力(TAOC)以评价机体氧化应激水平和抗氧化防御能力;测定血浆甘油三脂(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C)含量,评价机体脂代谢状况。结果与NGT相比,NEW-T2DM组SF、TF、Fe、LDL-C、TC、TG、MDA水平显著升高,TAOC明显降低;T2DM前期组SF、TF、Fe、TC、TG、MDA水平显著升高,TAOC明显降低。结论在糖尿病前期,机体的铁代谢平衡已被破坏,进而导致氧化抗氧化系统的紊乱和脂质过氧化,在NEW-T2DM组,表现尤为明显。因此,SF可作为早期预测糖尿病的一个重要指标。 Objective To evaluate the iron metabolism, oxidative stress and antioxidative state in type 2 diabetic patients. Method By oral glucose tolerance test, all enrolled subjects were divided into three groups: normal glucose tolerance (NGT), per-diabetes (IFG/IGT) and newly diagnosed type 2 diabetes (T2DM).Plasma ferritin, transferrin and free iron were tested to estimate the body iron status. Plasma malondialdehyde (MDA) and total anti-oxidative capability (TAOC) were chosen as biomarkers of oxidative stress. Plasma triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) were also determined. Results Compared with NGT group, plasma ferritin, transferrin, free iron, LDL-C, TG, TC and MDA were elevated significantly in T2DM and pre-diabetes group while TAOC was decreased significantly in T2DM group. Conclusion The iron status was disturbed in subjects with pre-diabetes, leading to oxidative stress and destruction of antioxidant defense system. This situation became worse in subjects with T2DM. Plasma ferritin may be an important index to anticipate the progression of type 2 diabetes.
