摘要
目的探讨宫颈鳞状细胞癌(SCC)组织碱性成纤维细胞生长因子(bFGF)表达与局部血管淋巴管生成、转移及预后的关系。方法采用免疫组织化学SP法检测28例正常宫颈上皮(NCE)、36例宫颈上皮内瘤变(CIN)和68例SCC组织bFGF表达,并检测Ki-67增殖指数(PI)、CD34微血管密度(MVD)及VEGFR-3微淋巴管密度(LMVD)。结果从NCE到CIN再到SCC,bFGF表达水平、PI、MVD及LMVD均显著升高(P<0.05)。bFGF在SCC组表达与MVD显著正相关,并与盆腔淋巴结转移及脉管浸润密切相关(P<0.05),而与患者年龄、FIGO分期、组织学分级、间质浸润深度、预后、PI及LMVD无关(P>0.05)。SCC有盆腔淋巴结转移及脉管浸润者,其bFGF表达水平分别显著高于无盆腔淋巴结转移及无脉管浸润者(P<0.05)。结论bFGF表达与SCC局部微血管密度及转移状况呈正相关,提示bFGF可能在SCC局部血管生成及浸润转移过程中发挥重要作用。
Objective To investigate the expression of basic fibroblast growth factor (bFGF) and their correlation with regional angiogenesis, lymphangiogenesis, metastasis, and prognosis in cervical squamous cell carcinoma. Methods The expression of bFGF protein, proliferation index (PI) labeled by Ki-67, microvessel density (MVD) labeled by CD34 and lymphatic microvessel density (LMVD) labeled by VEGFR-3 protein in 28 cases with normal cervical epithelium (NCE), 36 cases of cervical intraepithelial neoplasm (CIN) and 68 cases with squamous cell carcinoma of cervix (SCC) were detected by immunohistochemistry SP method. Results The expression of bFGF, PI, MVD, and LMVD all increased remarkably from NCE to CIN and then to SCC groups accordingly (P〈0.05). In SCC group, the expression of bFGF positively with MVD, pelvic lymph node metastasis and intravascular infiltration (P〈0.05), but not correlated with patient's age, FIGO staging, histological grading, stroma involvement, prognosis, PI and LMVD (P〉0.05). In cases with pelvic lymph node metastasis and intravascular infiltration, the expression of bFGF was significantly higher than that in those without the conditions mentioned above (P〈 0.05). Conclusion The expression of bFGF in SCC group positively correlats with MVD and metastasis. bFGF may play an important role in regional angiogenesis, cancer invasion and metastasis in squamous cell carcinoma of cervix.
