经眶上缘外侧锁孔入路鞍区的内镜解剖学观察 被引量:1

Endoscopic Anatomy of the Operating Spaces in Saddle Area via the Superolateral Orbital Rim Keyhole Approach

摘要 目的:为鞍区手术提供入路及相关内镜解剖学资料。方法:本组采用15具10%甲醛溶液固定的成人尸头和1例新鲜尸头标本,经眶上缘外侧锁孔入路用显微镜和内镜(0°镜、30°镜),对鞍区各间隙进行解剖观察并录像。结果:经眶上缘外侧锁孔入路开颅可达到经典翼点入路对骨窗的基本要求,通过使用内镜不同角度的操作,可清晰的显露间隙Ⅰ、Ⅱ、Ⅲ、Ⅳ及Willis′环,尤其对重要穿通血管的显露更明确。结论:经眶上缘外侧锁孔入路创伤小,暴露好;而内镜在鞍区的应用可弥补手术显微镜的不足,对显微外科手术起到重要的辅助作用。 Objective:To provide anatomic data concerning operative approach and endoscope for the surgical procedure in saddle area. Methods : Fifteen heads of adult cadavers soaked in formalin and one fresh adult cadaver head were studied via the superolaterai orbital rim keyhole approach. The anatomic structures of every interspace in saddle area were observed by microscopy and endoscopy (0° and 30° hard) alternatively and recorded. Results: The basic information obtained via the superolateral orbital rim keyhole approach was comparable to that obtained via the classic pterion approach. The spaces Ⅰ ,Ⅱ,Ⅲ and Ⅳ and Willis'circle can be clearly displayed by an endoscope at various angles, especially for perforating branches. Conclusions: The superolateral orbital rim keyhole approach was a minimal invasive procedure with better exposure. Application of endoscope in saddle area will make up for the deficient of operating microscope and it is an important assistance to microsurgery.
