氩激光治疗黄斑裂孔性视网膜浅脱离的疗效

摘要 目的评价氩激光治疗黄斑裂孔性视网膜浅脱离的疗效。方法对33例33只眼黄斑裂孔性视网膜浅脱离施行氩激光治疗,激光波长514.5 nm,功率0.2~0.3 W,光斑直径75~100μm,照射时间0.1 s,360°封闭,光凝斑反应1~2级。比较黄斑裂孔性视网膜浅脱离封闭的成功率及光凝前后视力。结果25只眼一次光凝成功(占75.8%);6只眼二次光凝成功(占18.2%),2只眼二次光凝后未能复位,行玻璃体切割联合气液交换注入全氟丙烷混合气体后复位。随访12~17个月,平均(14.5±2.3)个月,光凝后视力提高5只眼(15.2%),视力不变24只眼(72.7%),视力下降4只眼(12.1%)。结论氩激光治疗黄斑裂孔性视网膜浅脱离成功率高且能有效保持患眼视力,尤其适用年老体弱患者,可于门诊开展治疗。 Objective To evaluate the effects of the argon laser photocoagulation for limited retinal detachment with macular hole. Methods Thirty three cases of limited retinal detachment witla macular hole (33 eyes) were treated by argon laser photocoagulation. Their healing rate and the visual acuity before surgery and after surgery were compared. Results Twenth five eyes obtained retinal reattachment after the first laser surgery(75.8% ) , 6 eyes succeeded in the second laser surgery( 18. 2% ) and 2 eyes failed, but succeeded after vitrectomy. All cases were followed 12-17 months (14. 5 ± 2. 3 months), postoperative vision showed an increase in 5 eyes( 15.2% ) and no changes in 24 eyes(72. 7% ) and decreased in 4 eyes( 12. 1% ). Conclusions It was shown that argon laser photocoagulation is an effective method in treating limited retinal detachment with macular hole, and benefit to retain vision, especially to olderly or weak patients and convenient for clinic treatment.
