MSCT诊断髋关节创伤的价值

The Value of Multislice Spiral Computer Tomographic Scanning in Trauma Patient of Hip Joint

摘要 目的:探讨MSCT在髋关节创伤诊断中的价值。材料和方法:分析39例创伤患者的44个髋关节的MSCT影像,比较MPR和VR重建技术对髋关节创伤显示的差异。结果:MSCT共发现44个关节78处骨折,59处骨折有移位;30个髋臼关节面和11个股骨头关节面塌陷或碎裂,32个关节内存在骨碎片。其中MPR显示骨折的细节明显优于VR,显示了40(90.9%)个关节骨折,其中55(93.2%)处骨折移位;39个关节面(95.1%)碎裂或塌陷,29个关节(90.6%)内存在骨碎片;VR对于骨折线的走行和骨折移位情况显示较好,对骨碎片显示不佳。MSCT对于骨折分类的确定优于X线平片。结论:对于髋关节创伤的诊断,MPR对于确定骨折类型和显示骨折细节非常有价值,应该常规应用。 Objective: To explore the value of multislice spiral computer tomographic scanning in trauma patient of hip joint. Methods: The plain film and MSCT data of 44 joints in 39 patients with trauma of hip joint were analyzed. Compare the diagnositic value of MSCT and X-ray film on trauma of extremitiy joint. Results: MSCT features included: 78 fractures of 44 joints, 59 fractures displacing, 30 acetabulum and 11 femoral heads collapse or fragment. MPR shows 40 fractures in the joints, 55 fractures displacing, 39 articular surfaces collapse, fragments in 29 joints. Comparing with VR, MPR is much better in showing details of fracture, and VR is much better in showing the fracture line and displacement. Conclusion: MSCT is a very useful method in the diagnosis of the trauma of hip joint, particularly using MPR and VR.
