食管贲门双源癌的外科治疗

Surgical therapy for multiple-primary carcinoma of esophagus and cardiac

目的探讨食管贲门双源癌的诊断和外科治疗方法,提高诊断率和手术效果。方法回顾性分析我科自2004年1月～2007年12月手术治疗的15例食管贲门双源癌的临床病例资料。结果术前确诊13例,均经内腔镜检查活组织病理检查和X线检查。诊断率86.7%(13/15),术后确诊2例(2/15)。胸中段肿瘤5例,胸下段肿瘤10例,本组15例均行手术治疗,手术无死亡。结论本病术前要行内镜检查病理检查和X线钡餐检查来确诊,减少漏诊率。并积极进行外科治疗,有望改善治疗效果。

Objective To explore the diagnosis and operative treatment of multiple - primary carcinoma of esophagus and cardiac, improve the operative measure and enhance the confirmed diagnosis rate of preoperation. Methods The clinical datas of 15patients with multiple carcinoma of esophagus and cardiac were analyzed retrospectively from Jan. 2004 to Dec. 2007. Results Thirteen patients were diagnosed confirmatively in preoperation including 13 through encloscope and X - ray examination. The other 2 patients were diagnosed confirmatively in postoperation. There were 5 cases intermediate thoracic esophageal carcinoma and 10 coses lower thoracic esophageal carcinoma in the group. None of them died during operation for all patients. Conclusions Encloscope and barium meal are effective measure for confirmed diagnosis of esophagus and cardiac carcinoma preoperatively. Multiple - primary carcinoma need benoticed especially in examination. Surgical is still the top priority in the selection of treatment.
