血管内乳头状内皮细胞增生

Intravascular papillary endothelial hyperplasia: a case report

报告1例血管内乳头状内皮细胞增生。患者女,42岁。左手小指近端肿物伴疼痛半年余。皮肤科检查:左手小指近端见一隆起于皮面、直径约2 cm 的结节,结节表面肤色正常,硬度适中,活动度小,与皮肤无明显粘连,压痛明显。皮损组织病理检查:皮下见大的血管管腔,并从管壁伸出乳头状突起,中轴为纤维组织增生,管腔内大量红细胞,并见大量内皮细胞增生,形成网状的血管管腔,可见深染略有多形的核,但无明显的核非典型性及有丝分裂象,团块中央部位可见含铁血黄素沉积,弹性纤维染色呈阳性。免疫组化染色 CD31、CD34、FⅧ阳性。组织病理诊断:血管内乳头状内皮细胞增生。

A case of intravascular papillary endothelial hyperplasia is reported. A 42-year-old woman presented with a painful swelling on the proximal end of little finger for half a year. Physical examination showed a nodule in a size of 2 centimeters in diameter, with normal skin color and tenderness. Histopathological examination revealed subcutaneous large vascular cavities composed of proliferating endothelial cells with hyperplastie fibrin and a large number of erythroeytes within the vessels. The nucleli of endothelial cells appeared dark-stained and pleomorphie but lacked atypia and mitotic figures. Hemosiderin deposition in the stroma could be seen and the Verhoeff＇s elastic fiber stain was positive. Immunohistochemistry showed the positive staining for CD31, CD34 and FⅧ.
