高频彩超诊断较小乳腺癌 被引量:3

High Frequency Ultrasonography and Color Doppler Flow Imaging in the Diagnosis of Small Breast Cancers

摘要 目的:探讨高频彩超对较小乳腺癌(最大直径≤15mm)的诊断价值及误诊原因分析。方法:对82例女性患者术前分别进行二维超声检查、彩色多普勒超声及频谱多普勒超声检查。观察病变的大小、形态、边缘、内部回声、后方回声、钙化、内部及边缘血流信号、峰值流速、阻力指数等。结果:本组病例共有82个病灶,均经手术或穿刺病理证实为乳腺癌。术前高频彩超对较小乳腺癌(最大直径≤15mm)的诊断符合率为81.7%(67/82);误诊为良性病变9个,误诊率11%(9/82),其中导管内癌2个;术前超声难以判断良恶性者6个,其中导管内癌2个。结论:高频彩超对较小乳腺癌具有较高的诊断价值,但对导管内癌诊断较困难。术前配合实时超声引导下的乳腺病变穿刺活组织检查术可进一步提高较小乳腺癌的术前诊断率。 Objective: To evaluate the value of high frequency and color Doppler ultrasonography in the diagnosis of small breast cancers. Methods: Totally, 82 female patients with small breast cancers were examined by high frequency and color doppler ultrasonography before operation. The size, echogenicity, echo, shape, border, calcification, and vascularity on breast le sions were observed by ultrasonography. Results: Eighty-two female patients were proved by pathology after operation or biopsy. Of them,64 lesions were infiltrated ductal carcinoma,7 ductal carcinomas,4 invasive lobular carcinomas, 1 nerve endocrine carcinoma in ductal, 1 gland ductal cancer, 1 gland tube kind cancer, 1 papillary carcinoma in ductal and medullary of ductal epithelial. The accurate rate of high frequency and color doppler ultrasonography was 81.7% (67/82). Misdiagnosis rate was 11 %(9/82),2 was ductal carcinoma in situ. No diagnosis was 6, 2 duetal carcinoma in situ. Conclusion: High frequency and color doppler ultrasonography is useful diagnostic method in small breast cancers. Sonography guided core needle biopsy can increase diagnostic rate before operation.
