影响肾癌预后的因素

Prognostic factors of renal cell carcinomas

目的探讨影响肾癌预后的因素。方法对401例肾癌患者,就20项临床病理因素作Kaplan-Meier单因素分析和COX多因素分析,所有数据用SPSS11.5统计软件处理。结果单因素分析,年龄、性别、病理类型等12项因素对生存率有影响(P<0.01)。COX多因素分析,仅有年龄、肉瘤样变等7项因素是影响肾癌预后的独立因素(P<0.05)。结论老龄、有肉瘤样变、全身症状、腔静脉瘤拴、淋巴结转移、远处转移、TNM高分期肾癌病人预后差。

Objective To explore the prognostic factors of renal cell carcinomas with different symptoms. Methods A total of 401 cases were included in this study. 20 clinical-pathological factors were analysed with the Kaplan-Meier univariate method and Cox multivariate analysis. All data were assessed with SPSSll. 5. Results On univariate analysis age, gender, sarcomatoid component, histologic subtypes, grade, symptoms, venous thrombus, tumor size, lymphatic metastasis, distant metastasis, TNM stage, and ECOG performance status were significant prognostic factors （P〈0.01）. On multivariate analysis age, sarcomatoid component, systemic symptoms, venous thrombus, lymphatic metastasis, distant metastasis, and TNM stage remained independent prognostic factors （P〈0. 05）. Conclusion Worse prognosis is prone to patiens with old age, sarcomatoid component, systemic symptoms, venous thrombus, lymphatic metastasis, distant metastasis and high TNM stage.
