摘要
目的检测HLA-DR抗原及共刺激分子CD80/CD86在不同程度干眼患者球结膜上皮细胞中的表达。方法通过结膜印迹细胞法获取球结膜上皮细胞,通过流式细胞仪定量分析HLA-DR和共刺激分子CD80/CD86的表达。结果轻度及重度干眼患者中HLA-DR的表达较正常对照组均有明显上调;重度组中Sjgren综合征(SS)组均较非SS组表达增加。重度组干眼患者共刺激分子CD80/CD86的表达较正常对照组差异无统计学意义。结论HLA-DR在不同程度干眼患者结膜上皮细胞中的表达情况反映了疾病的进展。结膜上皮细胞可能作为非专职抗原递呈细胞引起T细胞激活。
Objective Several hypothesis have been proposed to explain the pathogenesis of dry eye disease. However,the actual mechanism is still unclear. A growing evidence points out that inflammation is a key component of dry eye. This study aimed to examine the expression of HLA-DR antigen and co-stimulatory molecules CD80/CD86 in the bulbar conjunctiva epithelial cells in different degrees of dry eye patients. Methods 104 eyes of 55 patients with dry eye were collected based on OSDI questionnaire survey,tear break-up time(BUT) ,Schirmer test and red stain. The patients were divided into mild dry eye group (44 eyes) and severe dry eye group (60 eyes) with Sjogren' s syndrome (SS) and Non-SS. Thirty-two normal eyes from 16 subjects with matched age and gender were as the normal control. Impression cytology was used to collect conjunctiva epithelial cells from dry eye group and normal control group. HLA-DR and co-stimulatory molecules B7 positive cells were detected and analyzed by flow cytometry. Results The expression of HLA-DR in conjunctiva epithelial cells was (51.11±9.58 ) % in severe dry eye patients,showing a significantly elevation in comparison with mild dry eye patients(31.77±9.11 )% and normal controls (6.83±3.6) % (t =28. 039 ,t=11. 165 ,P 〈0.01 ) ,and that in mild dry eye patients was obviously increased than the normal controls(t = 15. 058,P 〈0. 01 ). The expression of HLA-DR in SS(58.25±10. 40)% was higher than that in Non-SS (51.97 ± 7.72)% in severe dry eye patients(t = 2. 167,P 〈 0.05). There was not a significant difference in co-stimulatory molecules CD80 and CD86 between severe dry eye patients and normal controls (t = 0. 810,P = 0. 427;t = 0. 822,P = 0. 420). Conclusion HLA-DR expression pattern in conjunctiva epithelial cells in different degree of dry eyes appears to reflect the progression of disease. Conjunctiva epithelial ceils can induce the activation of T-cells as a "non-professioner" antigen passing cells.
