摘要
目的:探讨细胞角蛋白5/6(CK5/6)及表皮生长因子受体(EGFR)在浸润性乳腺癌组织中的表达及其意义。方法:采用免疫组织化学EnVision二步法检测163例浸润性乳腺癌组织CK5/6及EGFR的表达。结果:CK5/6及EGFR在浸润性乳腺癌组织中的表达率分别为16.6%(27/163)及41.1%(67/163)。CK5/6及EGFR的表达与患者年龄和肿瘤大小均无明显相关性,CK5/6的χ2值分别为0.418和0.032,P值分别为0.518和0.858;EGFR的χ2值分别为0.548和1.775,P值分别为0.459和0.183。CK5/6及EGFR的表达与组织学分级、TNM分期及淋巴结是否转移存在明显相关性,CK5/6的χ2值分别为32.616、63.781和9.847,P值分别为0.000、0.000和0.002;EGFR的χ2值分别为19.057、19.242和19.508,P值均为0.000。组织学分级越高及TNM分期越高者CK5/6及EGFR的表达越高。淋巴结转移者CK5/6及EGFR的阳性率高。CK5/6和EGFR在ER、PR及HER2阴性乳腺癌者(三联阴性)表达率高。结论:乳腺癌检测CK5/6及EGFR,同时结合ER、PR和HER2的表达情况,具有重要的临床意义。三联阴性及CK5/6阳性和(或)EGFR阳性者为基底细胞样乳腺癌,临床预后差。
OBJECTIVE: To investigate the expressions and clinical significance of cytokeratin 5/6 (CK5/6) and epidermal growth factor receptor (EGFR) in invasive breast cancer. METHODS: The expressions of CK5/6 and EGFR were detected in 163 cases of invasive breast cancer by the immunohistochemical EnVision two-step method. RESALTS: The positive expression rates of CK5/6 and EGFR in invasive breast cancer were 16.6%(27/163) and 41.1%(67/163), respectively. The expressions of CK5/6 and EGFR were not related to patient's age and tumor size (X^2 values of CK5/6 were 0. 418 and 0. 032, P values were 0. 518 and 0. 858, X^2 values of EGFR were 0. 548 and 1. 775 and P values were 0. 459 and 0. 183, respectively). The expressions of CK5/6 and EGFR were correlated with the histological grade, TNM stage and lymph node status (X^2 values of CK5/6 were 32. 616,63. 781 and 9. 847, P values were 0.000,0.000 and 0.002, X^2 values of EGFR were 19.057, 19. 242 and 19. 508, respectively, and all the P values were 0. 000). There were high positive rates in high grade, high stage and lymph node positive patients. There were high positive rates of CK5/6 and EGFR in ER, PR and HER2 negative (triple-negative) breast cancer. CONCLUSIONS: Detections of CK5/6 and EGFR combined with ER, PR and HER2 have important clinical significance. The breast cancer with triple-negative, CK5/6 positive and (or) EGFR positive is defined to basal-like hreast cancer, and "those patients have poor clinical prognosis.
