荧光定量PCR检测外周血CK20 mRNA对子宫内膜癌微转移的诊断意义 被引量:1

摘要 目的:研究子宫内膜癌患者外周血中角蛋白20(CK20)mRNA的表达情况,并探讨荧光定量PCR(FQ-PCR)检测子宫内膜癌微转移的可行性及临床价值。方法:采用FQ-PCR方法对64例子宫内膜癌患者外周血、42例良性子宫病变患者及20位健康女献血员外周血中的CK20 mRNA表达水平进行检测;并同法检测20例术中切除的肿瘤组织的CK20 mRNA。结果:子宫良性病变患者及健康女性外周血中CK20 mRNA表达均<103copies/μL的检测下限;子宫内膜癌组织CK20 mRNA表达水平为(2.08±0.84)×106copies/μL,阳性率为100%。而64例子宫内膜癌患者外周血中CK20 mR-NA表达水平为(5.52±1.14)×104copies/μL,高表达率(阳性率)为40.6%(26/64)。CK20 mRNA表达水平与临床分期差异有统计学意义,P<0.05,与病理分级无关,P>0.05。结论:FQ-PCR可快速、灵敏地定量子宫内膜癌外周血CK20 mRNA的表达,对预测子宫内膜癌微转移以及判断患者预后具有一定指导意义。 OBJECTIVE: To study the expression of eytokeratin 20 (CK20) mRNA in peripheral blood of endometrial cancer and elucidate the feasibility and significance of detecting CK20 mRNA expression by fluorescent quantitative PCR (FQ-PCR). METHODS: FQ-PCR was used to detect the CK20 mRNA expres sion in the peripheral blood of 64 patients with endornetrial carcinoma, 42 patients with benign endometrial disease, and 20 healthy women. In addition, CK20 mRNA expression in the cancer cells from 20 patients with endometrial carcinoma was also detected. RESULTS: The expression of CK20 mRNA was not detected in peripheral blood in both healthy women and patients with benign endometrial disease. However, the expression level of CK20 mRNA in peripheral blood from endometrial cancer patients was (5.52 ± 1.14)× 10^4 copies/pal, and the expression positive rate was 40.6%(26/64). With the progress of clinical stage, the ex pression level of CK20 mRNA increased (P〈0.05), but it was not correlated with pathological type of the cancer, P〉 0.05. CONCLUSIONS: FQ-PCR is a rapid and sensitive method for quantitating CK20 mRNA. The detection of CK20 mRNA expression in peripheral blood of patients with endometrial carcinoma may be helpful to identify micrometastasis, monitor the progression of the disease, and observe the effect of clinical treatment.
