摘要
目的:探讨食管鳞癌组织中基质金属蛋白酶(MMP-9)和血管内皮生长因子C(VEGF-C)表达的临床意义,分析两者与食管鳞癌组织分化程度、TNM分期、淋巴结转移和预后的关系。方法:采用SP法检测86例食管鳞癌组织及42例食管正常黏膜组织中MMP-9和VEGF-C表达情况。结果:正常42例食管黏膜组织未见有MMP-9和VEGF-C表达。MMP-9在食管鳞癌组织中表达的阳性率为65.12%(56/86)。其中在高中分化和低分化食管鳞癌组织中,MMP-9阳性表达率分别为51.06%(24/47)和82.05%(32/39),χ2=9.01,P=0.0027;Ⅰ~Ⅱ期和Ⅲ~Ⅳ期患者分别为42.86%(15/35)和80.39%(41/51),χ2=12.90,P=0.0029;淋巴结转移组和无淋巴结转移组分别为75.81%(47/62)和37.50%(9/24),χ2=11.20,P=0.0008。食管鳞癌组织VEGF-C阳性表达率为52.32%(45/86)。在高中分化鳞癌和低分化鳞癌VEGF-C表达阳性率分别为38.30%(18/47)和69.23%(27/39),χ2=9.500,P=0.0116;Ⅰ~Ⅱ期和Ⅲ~Ⅳ期患者的VEGF-C表达阳性率分别为34.29%(12/35)和64.71%(33/51),χ2=10.37,P=0.0023;淋巴结转移组和无淋巴结转移组VEGF-C表达阳性率分别为61.29%(38/62)和29.17%(7/24),χ2=13.32,P=0.0002。Kaplan-Meier分析发现,MMP-9高表达和VEGF-C高表达患者的生存时间明显低于MMP-9低表达和VEGF-C低表达患者。结论:MMP-9和VEGF-C的表达和食管鳞癌关系密切,MMP-9及VEGF-C的高表达可预示食管鳞癌高恶性度,高转移能力和低生存期;联合检测两者的表达有助于判断食管癌患者的预后。
OBJECTIVE: To explore the expresions of matrix metalloproteinases 9 and vascular endothelial growth factor-C and their significance in esophageal sqamous earcinoma (ESC) tissues. METHODS: The expressions of MMP-9 and VEGF-C of 86 cases of ESC and those of 42 cases of normal esophageal mueosa were detected by the method of streptaridinperoxi dase. RESULTS: There were no expressions of MMP 9 and VEGF C in normal esophageal mueosa of the 42 cases. The expressivon rate of MMP-9 of ESC was 65. 12% (56/86). The expressions of MMP-9 of well-mediate differentiation and poor differentiation were 51.06% (24/47) and 82.05%(32/39) respectively, χ^2= 9.01, P=0.002 7; those of grade Ⅰ -Ⅱ and grade Ⅲ-Ⅳ were 42. 86% (15/35) and 80.39%(41/51) respectively, χ^2=12. 90, P=0.002 9; the expressions of MMPK 9 of lymph node metastasis and no lymph metastasis were 75.81% (47/62) and 37.50% (9/24) respectively, χ^2=11.20, P=0.000 8. Kaplan Meier showed the survival time of high expressions of MMP 9 and VEGF-C was lower than that of low expressions of MMP-9 and VEGF C. CONCLUSIONS: the expression rates of MMP-9 and VEGF-C are closely related to the esophageal squamous carcinoma. The more expressions of MMP-9 and VEGFC would be poorer differentiation, high metastasis and low survival rate, Their expressions appear to be well-characterized indicators of tumor prognosis and metastasis.
