肾透明细胞癌SCT多期增强表现与COX-2表达和肿瘤血管生成相关性的探讨 被引量:2

摘要 目的:探讨肾透明细胞癌(CRCC)的螺旋CT(SCT)多期增强表现与癌组织中环氧化酶-2(COX-2)表达及其微血管密度(MVD)之间的关系。方法:收集经手术病理确诊且术前有详细SCT资料的CRCC石醋包埋切片32例和正常肾组织切片10例,采用免疫组化SP法检测Ⅷ因子相关抗原和COX-2的表达。观察SCT图像上平扫、CMP、NP和EP时肿瘤密度及肿瘤最大强化程度等多期增强指标。结果:CRCC组织中,MVD明显高于正常肾组织,U=7.148,P=0.000;COX-2表达也高于正常肾组织,χ2=9.773,P=0.002。COX-2表达程度与MVD之间有显著相关性,rs=0.739,P=0.003。CRCC癌组织的最大强化程度、强化各期CT值与COX-2表达程度和MVD呈正相关。结论:CRCC的SCT多期增强表现可以无创伤评价肿瘤血管生成,在一定程度上反映COX-2的表达。 OBJECTIVE: To study the correlation between multiphase enhanced spiral CT features and the expression of cy clooxygenase 2(COX-2), and microvessel density (MVD) respec tively in clear renal cell carcinoma. METHODS: Ⅷ- related antigen,and the expressions of COX 2 in 32 patients with CRCC diag nosed by pathology and 10 normal renal tissues were examined im munohistochemieally by S P techniques. CT images were observed for the density on unenhanced scan, CMP, NP and EP, and the maximum enhancement extent. RESULTS: The positive expression rate of COX2 (U=7. 148, P =0. 000) and MVD (χ^2= 9. 773, P =0. 002) in CRCC were significantly higher than those in normal renal tissues. The expression of COX2 and MVD showed a positive corre lation in CRCC, rs=0. 739, P=0. 003. The density on CMP, NP, EP and maximum enhancement extent in CRCC showed a positive correlation with the expression of COX-2 and MVD respectively. CONCLUSION: The features of multiphase enhanced spiral CT in CRCC are helpful to assess the tumor vascularity and angiogenesis,and could reflect the expression of COX-2 to some extent.
