我院住院患者退药情况调查及分析

Analysis about drug repercussion of inpatients

目的调查我院住院患者退药情况。方法收集我院各科病区2008年6月-8月1929份退药申请单及电脑数据库病区退药金额数据,采用U检验,分析我院住院患者的退药特点。结果退药原因多见于患者出院(42.15%)、调整医嘱(22.71%)、患者转科(11.20%)、药物不良反应(8.55%)、患者用药不依从(6.84%)等。退药品种和例数统计,抗生素类(82种,1094例次)和调节水、电解质类药物(35种,582例次)位列前2名。我院不同病区的退药情况差异较大,内科共退药816例,退药金额占总退药金额42.03%,其中神经内科(10.27%)、心血管内科(9.40%)、呼吸内科(7.19%)排在前三位;外科共退药874例,退药金额占总退药金额35.61%,其中骨科(9.41%)、普外科(7.11%)、妇科(4.14%)排在前三位。其他科室共退药239例,退药金额占总退药金额22.36%,其中ICU退药金额占总退药金额13.08%。结论我院各病区均有退药现象,且退药量大,加强我院药品规范管理和临床合理用药势在必行。

Objective To investigate the drug repercussion of inpatients in our own hospital. Methods 1929 lists applying for drug repercussion and amount of money about drug repercussion in data base were collected. The characteristics and causes of drug repercussion were analyzed by U test. Results Discharge from hospital （42.15%）, adjustment about medication （22.71%）, transferring to another department （11.20%）, adverse drug reactions （8.55%）, and noncompliance of patients （6.84%） were most reasons for drug repercussion. Antibiotics （82 kinds, 1094 cases） and moderators of water and electrolyte （35 kinds, 582 cases ） were the top two sorts of retraced drugs. The situation of drug repercussion is different in different wards. There were altogether 816 cases of drug repercussion in internal ward, which occupied 42.03% of total amount of money about drug repercussion, of which neurology department （10.27%）, cardiology department （9.40%）, respiratory department （7.19%） were the top three. There were altogether 874 cases of drug repercussion in surgery ward, which occupied 35.61% of total amount of money about drug repercussion, of which orthopedics department （9.41%）, general surgery department （7.11%） and gynecology department （4.14%） were the top three. There were altogether 239 cases of drug repercussion in other wards, which occupied 22.36% of total amount of money about drug repercussion, of which ICU occupied 13.08%. Conclusion Drug repercussion occured in every ward of our hospital. It is imperative to enhance the standard management of drugs.
