经皮球囊二尖瓣成形术后患者血浆脑钠肽的变化意义

摘要 目的:探讨风心病二尖瓣狭窄(MS)患者经皮球囊二尖瓣成形术(PBMV)后血流动力学参数及血浆脑钠肽(BNP)的变化。方法:选择进行PBMV的MS患者22例(均为窦性心律),PBMV术前、术后测定肺动脉平均压(MPAP)及左房平均压力(MLAP),术前及术后1天经超声测定左房内径(LAD),二尖瓣口面积(MVA)及二尖瓣跨瓣压差(MVPG),另术前1天及术后5天分别取外周血以ELISA法测定血浆BNP浓度。结果:与术前相比,术后MVA明显增加(P<0.01),而MPAP,MLAP,LAD及MVPG则明显减小(P<0.05或0.01),同时BNP显著降低(P<0.01)。结论:PBMV能显著改善患者的血流动力学,基于此BNP则降低,BNP降低对风心病MS患者PBMV术后的疗效及预后有一定的预示作用。
