摘要
目的应用手术规划系统,探索虚拟现实技术在神经外科的应用价值和方法。方法术前采集10例颅内深部病变患者的CT、MRI、CTA、MRA、DSA影像资料输入Dextroscope工作站进行图像融合处理和三维重建,在虚拟影像上观察病变与周围组织的空间关系,设计手术入路、模拟手术过程进行骨窗开颅、颅骨磨除及肿瘤切除,并与实际术中所见进行比较。结果术中所见病变与周边重要神经血管结构的空间关系和病变解剖细节与术前模拟完全吻合,减少了手术时间。通过手术模拟,术者得以提前预演手术经过,为优化治疗方案创造了条件。结论虚拟现实手术规划系统有助于术前诊断、制定详细的手术计划、预先评价手术难度,提高手术安全性。是医疗教学和训练的得力工具。
Objective To evaluate the application of dextroscope virtual reality (VR) surgical planning system in neurosurgical field on our experience. Methods Image data of 10 patients were uploaded to the dextroscope workstation preoperatively between October and December of 2006. Patient-specific data sets from multiple imaging techniques (magnetic resonance imaging, magnetic resonance angiography, magnetic resonance venography, and computed tomography ) were coregistered, fused, and displayed as a stereoscopic 3-D object for movement and manipulation in realtime. Surgical planning of approaches and procedures were carried out and compared with the intraoperative findings. Results The preoperative 3D model is in high concordance with intraoperative conditions. It helps the operator to optimize his surgical planning in most minimally invasive way. In all procedures planned with the Dextroscope, the chosen surgical strategy proved to be the correct choice. Conclusions The VR technology is very helpful in preoperative planning, and plays an important role in neurosurgical training.
