侧脑室肿瘤的显微外科手术策略

Microsurgical strategies of lateral ventricle tumor

目的探讨侧脑室肿瘤的显微外科手术策略方法回顾性分析17例侧脑室肿瘤患者的临床表现、影像学特点和显微外科手术方式。根据肿瘤的部位和大小选择最佳的手术入路,本组均采用经脑皮层入路。显微外科技术切除肿瘤,7例应用神经导航,5例应用内窥镜辅助手术。结果肿瘤全切除11例,次全切除4例,部分切除2例。术后临床症状改善16例,死亡1例。结论根据侧脑室肿瘤的起源位置和生长方式选择最佳手术入路,经皮层入路是一种较好的选择。神经导航、内窥镜辅助显微外科手术运用,提高了肿瘤的切除程度,降低了术后并发症。

Object To discuss the microsurgical strategies of lateral ventricle tumor. Methods The clinical manifestations, imaging features and microsurgical managements of 17 patients were retrospectively analyzed. The optimal surgical approaches were selected according to tumor origin and size. The transcortical approaches were selected in all patients. All cases were undergone microsurgery, the neuronavigation was used in 7 cases and the neuroendoscope was applicated in 5 cases. Results Total resection was realized in 11 cases, subtotal resection in 4 cases and partial resection in 2 cases. Postoperative clinical symptoms were alleviated in 16 cases, died in 1 case. Conclusions Lateral ventricle tumors can be treated by careful selection of the approach according to tumor origin and development., the transcortical approach is one kind of better option. Application of neuronavigation and neuroendoscope assisted by microsurgery can enhance the resection rate and reduce postoperative complication.
