老年及老年前期风湿性心脏病的临床特点

The clinical characters of rheumatic heart disease in the elder

目的：研究老年及老年前期风湿性心脏病的临床特点。方法：对５２例４５岁以上的风心病病人与同期５３例１４～４４岁的风心病病人的资料进行对比分析。结果：显示老年及老年前期风心病起病隐袭，有风湿史者少，风湿活动少，病程长，合并症多，瓣膜病变重，发生二尖瓣钙化的病例多，左房增大明显，发生持续性心房纤颤例数多，与对照组比较，差别显著或非常显著。结论：了解老年风湿性心脏病的临床特点，采取积极的治疗措施，将有助于改善患者的预后。

Objective: The aim of this study was to elucidate the clinical characters of Rheumatic Heart Disease (RHD) in the elder. Methods: Fifty-two cases of RHD age over 45 years were analyzed. Their clinical characters were compared with those in control group (53 cases of 14-44 years old) in the same period. Results: Compared with the control group, the history of typical rheumatic fever and clinical evidances of active rheumatic fever were less, the course of illness was longer and the associated disease, such as atrial fibrilation and calcification of the mitral valve, were much more, and the mean left atrial diameter was longer in the older Group. The differences between the two groups were statistically significant. Conclusions: All these finding might be helpful in understanding the clinical characters of RHD in elder patients and it suggested that we should actively treat the elder patients with RHD.
