锁骨外端骨折与肩锁关节脱位治疗分析

Treatment and Analysis of Collarbone and Shoulder Fracture-dislocation

目的评价不稳定型锁骨外端骨折与Ⅲ度肩锁关节脱位治疗方法和疗效。方法对120例不稳定型锁骨外端骨折和80例Ⅲ度肩锁关节脱位患者分别行手术复位,克氏针张力带固定。喙锁间螺钉固定和锁骨钩钢板固定治疗。平均随访24个月。疗效参照Lazcano的术后疗效评定标准。结果优良率:克氏针张力带组为83%,喙锁间螺钉固定组90%,锁骨钩固定组为98%。相比之下,锁骨钩钢板固定符合生物力学要求,疗效好,螺钉固定组次之,克氏针张力带固定组较差。结论对不稳定型锁骨外端骨折与Ⅲ度肩锁关节脱位内固定应首选锁骨钩钢板。

Objective To evaluate the unstable fracture of the collarbone, m-degree shoulder dislocation and efficacy of treatment. Methods 120 cases of unstable clavicle fracture and the end of the 80 cases m-degree shoulder dislocation reduction surgery patients,respectively, Kirschner wire tension band llxation, coracoclavicular between the clavicle hook plate and screw fixation for the treatment. Conclusion The end of instability collarbone fracture and Ⅲ degree shoulder dislocation within the first choice should be fixed clavicle hook plate.
