棘突椎板连接处结构植骨在腰椎融合术中的应用 被引量:2

Application of Lumbar Interbody Fusion with the Join Structure of Spinous Process and Veterbral Plate in Lumbar Fusion Operation

摘要 目的探讨棘突椎板连接处结构椎间植骨加后路椎弓根钉棒系统进行腰椎融合治疗腰椎不稳及退行性疾病的疗效及融合效果,为临床提供一种较为理想的椎间植骨选择。方法将手术病例随机分为两组,A组为椎间行棘突椎板连接结构植骨,B组为椎间行自体碎骨粒植骨,两组均采取后路椎弓根钉棒系统固定,于术后3个月~3年进行随访,观察两组的手术效果、植骨融合率、椎间隙高度,以及记录两组手术时间及出血量,并进行比较。结果两组术后症状均明显改善,手术时间、出血量、植骨融合时间及融合率均无明显差异,但在维持椎间隙高度方面,椎间行棘突椎板连接结构组明显高于植碎骨粒组。结论在腰椎融合术中行自体棘突椎板连接结构椎间植骨效果确切,能够有效的维持椎间隙高度,是一种良好的椎间植骨选择。 Objective To explore the treatment outcome ot the bone conjuction structure in mtrebody fusion tor lumbar instability and other degenerative diseases and provid a resonable clinical choice for lumbar interbody fusion. Methods All cases are divided into two groups. A group interbody fusion with the join structure of spinous process and veterbral plate,B group with own bone drops. Both groups were applicated wih posterior pedicle screw fixation. All cases were followed-up for 3 months to 3 years,observed the operation outcome and hone graft fusion rate and hight of intervertebral space,and recorded the operation time and volume of blood lossing. All the datas were compared according to statistics. Results The symptom were obviously improved in all cases. There is no obvious difference in the results of operation time and volume of blood lossing and fusion time and fusion rates. AS the aspect of keeping interveterbral space ,the bights of the join structure was bigger than ones of the control group. Conclusion In the lumbar fusion operation,the outcome of interbody fusion with spinous process is surely well,can effectualy maintain the interveterbral space hight ,is a good interhody fusion choice.
