早期人工气道建立在重度有机磷农药中毒抢救中的作用

摘要 目的:探讨早期人工气道建立在重度有机磷农药中毒抢救中的作用。方法:22例口服有机磷农药中毒病人,给以多次充分洗胃,碘解磷定或氯解磷定等复能剂和阿托品综合治疗。病程2~7天发生急性呼吸衰竭。均立即行床旁气管插管并给以机械通气治疗。结果:治疗后19例恢复良好,2例于首次成功脱机后发生中间综合征(IMS),再次发生急性呼吸衰竭,给以建立人工气道并机械通气,抢救成功,1例心肺骤停病人抢救无效死亡。结论:早期人工气道建立可明显提高重度有机磷农药中毒抢救成功率。 Objective:To discuss the effect of establishment of artificial airway on severe organophosphate poisoning with respiratory failure(RF). Methods:22 cases of oral organic pesticide poisoning were repeatedly given the comprehensive treatment with gastric lavage, iodine pralidoxime or pralidoxime chloride and atropine. Acute respiratory failure occurred in 2-7 d. All accepted bedside emergency treatment with tracheal intubation immediately.Results:After treatment, 19 cases recovered well;2 cases had intermediate syndrome after success in weaning at first time and recovered after establishing artificial airway again ; 1 case died from cardiopulmonary arrest. Conclusion:Establishment of artificial airway could significantly improve the successful rescue rate of severe organophosphate poisoning.
