参附注射液在体外循环中对心肌的保护作用 被引量:2

摘要 目的观察参附注射液用于体外循环中对心肌保护的作用。方法选择体外循环手术76例,随机分为观察组(Ⅰ)39例,对照组(Ⅱ)37例。Ⅰ组:麻醉诱导时,体外循环转流(CPB)前,主动脉开放时分别静滴参附注射液0.6ml/kg;Ⅱ组:全过程不用参附注射液,监测心脏复跳情况,心电图、心律失常(房颤、室颤、房室传导阻滞)、有无使用电除颤、心电图S-T段改变及T波的变化,并记录两组病人年龄、心功能分级、体外循环转流时间及主动脉阻断时间。结果两组病人的年龄、心功能分级、体外循环转流时间及主动脉阻断时间比较均无统计学意义(P>0.05)。两组病人心脏复跳率:Ⅰ组100%,Ⅱ组86.49%;心律失常:Ⅰ组5%,Ⅱ组18.9%;电除颤:Ⅰ组2.5%,Ⅱ组13.5%,S-T段及T波改变:Ⅰ组2.5%,Ⅱ组16.2%。观察组心脏自动复跳率高,心律失常、电除颤少,心电图ST-T变化小(P<0.05)。结论参附注射液用于体外循环对心肌有良好的保护作用。 Objective To observe the protective effect of sbenfu injection on myocardium in CPB. Methods 76 CPB patients were derided into 2 groups : the observation group ( n = 39) and the control group ( n = 37). During the induction of anesthesia, shenfu injection was given ( 0.6 ml / kg ) when the aorta was opened before CPB in observation group . No Shengfu injection were given in control group. Heart resuscitation, electrocardiogram, arrhythmia ( atrial fibrillation, ventricular fibrillation, AVB), use of electric defibrillator, age of patients, heart function grade, time of CPB and aortic interception were inspected during the whole procedure. Results There were no significant difference in age, cardiac function grade, CPB time and aortic interrupted time between the two groups. (P 〉 0.05). The rate of heart resuscitation wasl00% in the observation group and 86.49% in the control group. The rate of arrhythmia was 5% in the observation group and 18.9% in the control group. Defibrillation were used in 2.5% patients in the observation group and 13.5% patients in the control group ; S -T and T - wave change were found in 2.5% patients in the the observation group and 16.2% in the in the control group. The rate of heart auto - resuscitation in the observation group was higher than that in the control group. The rate of arrhythmia and defibrillation , the changes of electrocardiogram ST - T in the observation group were lower than those in the control group (P 〈 0.05 ). Conclusion Shenfu injection has a good protective effect on myocardium in CPB.
