穿透性角膜移植后角膜厚度的变化及其意义

Corneal thickness changes and their prognostic value after penetrating keratoplasty

目的：了解穿透性角膜移植后角膜厚度的变化及其与移植结果之间的关系。方法：对３０例穿透性角膜移植患者的角膜厚度进行了动态观察。结果：穿透性角膜移植后１周时角膜厚度显著增加，此后逐渐下降，到３个月时接近正常水平。排斥组的角膜厚度显著高于非排斥组，其角膜增厚比其它排斥反应征象出现的要早２周左右。在出现这些排斥反应时，角膜厚度达到最高值。如排斥反应得以控制，其角膜厚度也逐渐下降。结论：穿透性角膜移植后角膜厚度的测定有预后意义。

Objectives:In order to determine how the corneal thickness changes and whether measurements of corneal thickness are of prognostic value after penetrating keratoplasty. Methods:We used the ultrasonic pachymetry to measure central corneal thickness at each regularly scheduled postoperative ophthalmic examination in 30 patients. Results:We found that the magnitude of the corneal thickness significantly increased at one week,then gradually decreased and was close to the normal level by about 3 months after surgery.The grafts in the rejection group were significantly thicker than that in the non-rejection group.Happening of the episode of corneal thickening was 2 weeks earlier than the other signs of the rejection reaction.When rejection reaction occurred,the corneal thickness reached the peak value;if rejection reaction was controlled,the corneal thickness gradually decreased. Conclusions:Corneal thickness measurements after penetrating keratoplasty are of prognostic value.
