痰热清注射液对ICU呼吸机相关性肺炎患者CRP、IL-6和TNF-α的调节作用

Effects of Tanreqing Injection on CRP,IL-6 and TNF-α of Patients with Ventilator Assoeiated Pneumonia

目的研究呼吸机相关性肺炎(VAP)患者血清C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子α(TNF-α)的变化及其与机械通气(MV)时间的相互关系,探讨痰热清注射液对VAP患者的治疗作用。方法将42例VAP患者随机分为两组,治疗组予基础治疗加用痰热清注射液,对照组单用基础治疗;治疗前、治疗后第1、3、5日检测血清CRP、IL-6、TNF-α动态变化,统计和比较MV时间。结果42例VAP患者治疗前血清CRP、IL-6、TNF-α较正常水平均明显升高;治疗组血清CRP、IL-6、TNF-α在治疗后5d较对照组明显下降。治疗组治疗后第5日血清CRP、IL-6和TNF-α与M V时间呈正相关。结论痰热清注射液在辅助VAP综合治疗中,通过抑制过度的炎症反应,部分恢复机体免疫功能,有助于VAP治疗而缩短MV时间。

Objective： To investigate the changes of serum levels of C reactive protein（CRP）, interleukin -6 （IL - 6） and tumor necrosis factor - α（TNF - α） of Tanreqing Injection treating on patients with ventilator associated pneumonia （VAP）and the correlations between these immune factors and the time of mechanical ventilation （MV）. Methods： 42 patients with VAP were randomly divided into the treatment group （with basic therapy and Tanreqing Injection）and control group（only with basic therapy） . Levels of CRP, IL - 6 and TNF - α were analyzed before the treatment and then analyzed again afterl, 3 and 5d of treatment. The times of MV were statistics. Results：Compared with the normal level, the serum levels of CRP, IL - 6 and TNF - α in all 42 patients with VAP were increased. The CRP, IL - 6 and TNF- α levels of treatment group after 5d of treatment was significantly decreased comparing with the levels of control group. Levels of CRP, IL - 6 and TNF - α after 5d of treatment were positively correlated with the times of MV. Comclusion ： In combined therapy of VAP, Tanreqing Injection regulates the inflammatory reaction obviously. It could restore the body＇s immune function, reduce the injury after VAP and shorten the times of MV.
