左氧氟沙星替代利福平治疗肺结核的临床研究

The research of levofloxacin substituting rifampicine in the treatment of pulmonary tuberculosis

目的观察左氧氟沙星替代利福平治疗初治肺结核患者的疗效和安全性。方法选择88例患者随机分为治疗组(2HEVZ/4HV)和对照组(2HREZ/4HR)。观察痰菌阴转、X线胸片好转情况及药物不良反应。结果治疗2个月时治疗组和对照组痰菌阴转率分别为87.2%和85.4%,满疗程6个月时痰菌阴转率分别为97.9%和95.1%。X线胸片肺内病灶吸收情况治疗组有效率97.9%,对照组有效率97.6%;全疗程结束时空洞闭合率,治疗组和对照组分别为73.1%、67.9%;满疗程时治疗组和对照组出现不良反应分别为18.8%和44.4%。结论左氧氟沙星具有良好抗结核作用,是一种使用安全、不良反应小的可以替代利福平的新一代抗结核药物。

Objective To observe the effects and security of levofloxacin substituting fifampicine in treating initial pulmonary tuberculosis patients. Methods 88 patients were randomly divided into treatment group（2HEVZ/4HV） and control group（2HREZ/4HR）, then the sputum negative conversion, CXR and drug adverse reaction were observed. Results In treatment group and control group the sputum negative conversion rate was 87.2% and 85.4% respectively after 2 months, then 97.9% and 95. 1% ater 6 months. According to CXR, the effective rate is 97.9% in treatment group and 97.6% in control group. After the full course of treatment , the pore closure rate was 73.1% in treatment group and 67.9% in control group. Meanwhile the rate of drug adverse reaction was 18.8% and 44.4% respectively. Conclusion There is only one small adverse reaction in the administration of luvofloxacin in clinical application. As a kind of antimicrobial levofloxacin can substitute rifampicine to treat pulmonary tuberculosis in elinical practice. It is safe and effective in the treatment of pulmonary tuberculo- sis, worthy to be promoted and administered widely in patients with such communicable disease.
