摘要
目的:探讨P53蛋白、血管内皮生长因子(VEGF)、微血管密度(MVD)在涎腺肿瘤中的表达及其与侵袭、转移等生物学行为的关系。方法:选取手术切除的涎腺肿瘤组织标本54例,采用免疫组织化学方法检测多形性腺瘤、黏液表皮样癌、腺样囊性癌中血管内皮生长因子(VEGF)、P53蛋白的表达和微血管密度(MVD)。结果:P53、VEGF、MVD在涎腺良恶性肿瘤中的表达差异有显著性(P<0.05);P53、VEGF的表达和MVD值与临床分期及是否区域或远处转移有关,而与肿瘤大小、部位无关。等级相关分析显示:随着P53表达水平的增高,MVD增加,呈显著正相关。本实验结果还表明,P53表达水平和MVD值随着VEGF表达程度的增高而增加。结论:P53蛋白、VEGF表达和MVD在涎腺肿瘤的发生、发展中起着重要作用。P53、VEGF和MVD可作为判断涎腺肿瘤生物学行为及预后的指标。P53与VEGF和MVD明显正相关,突变型P53的表达在涎腺肿瘤血管生成过程中具有重要意义。
Objective :To investigate the relationship between the expressions of vascular endothelial growth factor (VEGF) , P53 protein and the microvessel density (MVD) and invasion, metastasis in salivary gland tumors. Methods : 54 cases of surgical specimens of salivary gland tumor from two hospitals were selected. Use immunohistochemical method to analyze the expressions of P53 protein, VEGF and MVD in three salivary gland tumors. Resuits:In 54 cases of salivary gland tumors, the positive rates of P53, VEGF and MVD in malignant gland tumors were significantly higher than that in benign salivary gland tumors ( P 〈 0.05 ). The positive rates of P53, VEGF and the MVD were related to the nodal metastasis and the clinical substage( P 〈 0.05 ), and have no relation to the sites and sizes of the tumors. MVD in P53 and VEGF positive group was higher than that in their negative group(P 〈 0.05 ). Spearman rank correlation analysis showed that there was a positive corelatiou between VEGF and MVD. The same results were found between VEGF and P53, P53 and MVD. Conclusion: P53 and VEGF, MVD play a key role in occurrence and development of salivary gland tumor, and serve as an indicator of malignancy and devel- opment of salivary, gland tumor. P53, VEGF and MVD may be sen,ed as the predicators for the biological behavior and the prognostic factor of salivary gland tumor, p53 gene might play important role in promoting tumor angiogenesis.
