目的:检测Bmi-1在正常胃粘膜、胃癌及癌前病变中的表达,分析探讨Bmi—1与核增殖抗原ki-67和凋亡抑制基因Bcl-2之间的关系,以及Bmi-1^+细胞与Bcl-2^+/ki-67^-细胞分布之间的关系,探讨其与胃癌临床病理因素的关系及意义。方法:采用免疫组化Envision法检测162例胃癌及配对正常胃粘膜中Bmi-1和ki-67的表达,采用免疫组化双染法观察Bcl-2^+/ki-67^-细胞的分布。结果:胃癌组织中Bmi-1表达(52.5%)明显高于正常胃粘膜(21.6%),P〈0.05;胃癌中Bmi-1表达与Lauren分型、Borrmann分型和胃癌临床病理分期有关(P〈0.05)。胃癌中ki-67表达与Borrmann分型有关(P〈0.05)。Bcl-2在胃癌中的表达与胃癌Lauren分型有关(P〈0.05)。在胃癌组织和肠化生粘膜中,Bmi-1与Bcl-2呈正相关(P〈0.05)。结论:胃癌及其癌前病变中Bmi-1表达可能与细胞增殖、凋亡和癌变有关。在胃癌和肠化生粘膜中Bmi-1和Bcl-2表达的相关性以及Bmi-1^+细胞和Bcl-2^+/ki-67^-细胞分布之间的关系提示Bmi-1与胃癌的发生密切相关。
Objective: To investigate the expression of Bmi-1 in normal gastric mucosa, gastric carcinoma and precancerous lesions; to analyze the correlation of Bmi-1, ki-67 and Bcl-2 with the distribution of Bmi-1(+) cells and Bcl-2 (+)/ki-67 (-) cells; and to explore the correlation between Bmi-1 expression with clinicopathologic features of gastric carcinoma. Methods: Envision immunohistochemistry and immunohistochemical double-labelling staining were used to detect the expression of Bmi-1, ki-67, Bcl-2 and the distribution of Bcl-2 (+) /ki-67 (-) cells in 162 gastric carcinoma specimens. Results: The expression of Bmi-1 in gastric carcinoma specimens (52.5%) was significantly higher than in normal gastric mucosa (21.6%) (P〈0.05). The expression of Bmi-1 was related to Lauren's classification, Borrmann classification and clinical pathological stage (P〈 0.05). The expression of ki-67 was related to Borrmann classification in gastric carcinoma (P〈0.05). The expression of Bcl-2 was related to Lauren's classification (P〈0.05). In gastric carcinoma and intestinal metaplasia tissues, Bmi-1 expression was positively correlated with Bcl-2 (P〈0.05). Conclusion: The variation of Bmi-1 expression in gastric carcinoma and precancerous lesions might be involved in cell proliferation, apoptosis and cancerization. In gastric cancer and intestinal metaplasia, Bmi-1 is significantly associated with progression of gastric carcinoma.
